      目的?了解流感嗜血杆菌耐药率变化及多位点序列分型,进行同源性分析,减少医院感染和多重耐药菌株的发生。方法?收集解放军总医院第六医学中心2015—2022年临床分离的108株流感嗜血杆菌,使用K-B法进行药物敏感性试验;采用多位点序列分析进行序列分型(sequence typing, ST);采用MEGA 7.0软件的Neighbor-Joining法和Phyloviz 8.0软件的goeBURST算法构建系统发育树和最小生成树分析菌株同源性。结果?近几年流感嗜血杆菌对氨苄西林、复方新诺明耐药率>60%;108株菌中,5株未能与数据库匹配,103株菌有20种ST型别,主要为ST-487(58株,占53.70%)和ST-147(19株,占17.59%);系统发育树分为2组,除ST-834和ST-1242 2种型别分布在Ⅱ组外,其余型别在Ⅰ组;最小发育树提示2大分支ST-487和ST-147都与ST-103有相关性。结论?院内分离的流感嗜血杆菌多数菌株存在亲缘进化关系;多重耐药的流感嗜血杆菌在我院不同科室相继检出,提示编码耐药基因的质粒在病区间传播。

      Objective To investigate the variation in drug resistance rate and multilocus sequence typing of Haemophilus influenzae and analyze homology to reduce hospital-acquired infections and multidrug-resistant strains. Method?One hundred and eight strains of H. influenzae clinically isolated from the Sixth Medical Center of Chinese PLA General Hospital between 2015 and 2022. Drug sensitivity tests were conducted using the K-B method, and multilocus sequence analysis was used to sequence typing. We utilized the Neighbor Joining algorithm of MEGA7.0 software and the goeBURST algorithm of Phyloviz8.0 software to construct phylogenetic trees and a minimum spanning tree to analyze strain homology. Results?In recent years, the resistance rate of H. influenzae to ampicillin and compound sulfamethoxazole has exceeded 60%. Among the 108 strains of bacteria, 5 were not matched with the database, and 103 strains showed 20 ST types, mainly ST-487 (58 strains, 53.70%) and ST-147 (19 strains, 17.59%). The phylogenetic tree analysis was divided into 2 groups, with ST-834 and ST-1242 types distributed in Group II, and the remaining types in Group I. The minimum developmental tree analysis indicated that both branches ST-487 and ST-147 are correlated with ST-103. Conclusion?Most strains of H. influenzae isolated in hospital show phylogenetic relationships. Multidrug-resistant H. influenzae have been detected in different departments, suggesting that the drug resistance genes are being transmission between affected areas.
