      [摘要]?目的?了解2018—2021年北京地区流行的水痘-带状疱疹病毒(varicella-zoster virus, VZV)的基因型别特征,为水痘防控工作提供科学依据。方法?纳入2018—2021年我中心收治的157例水痘病例,检测VZV核酸,对阳性样本的VZV基因组的开放阅读框(open reading frame, ORF)22、ORF38、ORF54和ORF62区进行PCR扩增和测序,通过分析ORF22的6个单核苷酸多态性(single nucleotide polymorphism, SNP)确定病毒分支,评估ORF38(PstI)、ORF54(BglI)和ORF62(SmaI)的限制性酶切位点,确定野生株和疫苗株。结果?157例水痘患者中有91例VZV阳性,其中39例ORF22区测序成功,其与VZV clade 2基因型的代表株VOka的6个SNPs完全相同。亲缘关系分析发现,此39株与clade 2代表株均位于同一分支。通过比对ORF22序列发现仅有1条存在A37990G突变。在上述39例样本中ORF38、ORF54和ORF62全部测序成功28例,通过酶切位点分析发现,本研究获得了28株病毒酶切位点均为 PstI+BglI+SmaI+-,与既往我国报道的clade 2基因型野生株相同。结论?2018—2021年北京地区临床发现VZV毒株均为clade 2基因型野生株,未发现疫苗株感染病例。本研究提示加强原有疫苗接种仍可起到有效预防VZV感染的作用。

      [Abstract]?Objective?To analyze the molecular characteristics of varicella-zoster virus (VZV) circulating in Beijing from 2018 to 2021 and to provide suggestions for varicella prevention and control. Methods One hundred and fifty-seven varicella patients admitted to Fifth Medical Center of PLA General Hospital from 2018—2021 were enrolled in this study. All these patients were tested for VZV nucleic acid. PCR amplification and sequencing were performed in the open reading frame (ORF) 22, ORF38, ORF54 and ORF62 regions of the VZV genome in positive samples. Viral clades were identified by analyzing 6 single nucleotide polymorphisms (SNPs) of ORF22. The restriction enzyme cutting sites of PstI, Bg1I and SmaI in ORF38, ORF54 and ORF62 were respectively searched. Results Among 157 patients with varicella, 91 were VZV positive (CT≤ 35). Thirty-nine ORF22 sequences were obtained. We found that 6 SNPs of ORF22 were the same as the VOka strain (AB097932.1), a reepresentative strain of VZV clade 2. Phylogenetic analysis showed that these 39 ORF22 sequences were at the same clade with VOka, and only 1 sequence had an A37990G mutation. Of the 39 samples, 28 ORF38, ORF54 and ORF62 were sequenced successfully. By analyzingrestriction enzyme cutting sites, we found that 28 VZV strains obtained from samples were all PstI+BglI+SmaI+-, which were the same as the clade 2 genotype wild strain previously reported in China. Conclusions Our results suggests that the main VZV strains circulating in Beijing from 2018 to 2021 belong to clade 2. No case was infected with vaccine strain. This study suggests that strengthening the vaccination can effectively prevent VZV infection.
